Do you wish to be represented for the purposes of collective bargaining by SEIU-UHW (UNITED HEALTHCARE WORKERS-WEST), NEITHER, OR NUHW (NATIONAL UNION OF HEALTHCARE WORKERS). To which the choice of answers shall be "SEIU-UHW (UNITED HEALTHCARE WORKERS-WEST)" or "Neither", or "NUHW (NATIONAL UNION OF HEALTHCARE WORKERS)." If a majority of the professional employees in Voting Group "A" vote "yes" to the first question, indicating their wish to be included in a unit with nonprofessional employees, they will be so included. Their votes on the second question will then be counted together with votes of the nonprofessional employees. If the professional employees in Voting Group "A" vote against inclusion, they will not be included with nonprofessional employees.
