Western Ave., Chicago, Illinois and 966 W. 21st St., Chicago, IL consisting of all full time and regular par time non professional empoyees in the following classification: OB Cases Manager, CFCM Case Manager, CFCM Outreach Workers, Clerical Assistants, Doula Prenatal Evaluators, Prenatal Home Visitors, Program Support Clerks, Financial Evaluators, IDPA (onsite) Enrollment Coordinators, Kid Care, Patient Service Representative, Receptionis, Health Advocates, Postpartum Home Visitors, Home Visitors, Ounce of Prevention Interviewers, Facility Mantenance Aides, Medical Assistant, Patients Referral Coordinators, Medical Records, Clerks, Purchasing & Inventory Coordinators, Lead Medical Records Clerk Lead Receptionist/Patient Services, Representative, Lead Medical Assistant.
